Payment and Insurance Policies
As a courtesy, we will file your claim to your insurance company. Please be aware that it is ultimately your responsibility to be aware of your current coverage and benefits.
You will be responsible for any charges that are not covered by your insurance. We do not accept Medicare or Medicaid.
If you have new insurance, please fax us a copy of both sides of your card at least a day before your appointment.
Insurance verification for specialists can take up to 24 hours, and you may have to reschedule your appointment while we verify your new coverage.
You are welcome to use your credit/debit card, cash, or a money order for payment.
However, our billing system does not allow us to accept checks.
We understand that your time is valuable, and we are working hard to reduce wait times. Please help us by arriving on time for your appointment, and by giving us at least 24 hours of notice if you need to reschedule/cancel your visit.
Patients arriving more than 30 minutes after their scheduled appointment will be considered a "no show" and will be charged a $25 cancellation fee.
If our schedule allows, you may be given the option to wait for the next opening on the same day. However, please note that you will still be assessed the $25 fee, as we have to pay our staff to be idle during your original appointment slot.
We would rather help more patients relieve their pain than charge you fees, so please try your best to be timely.
Please note that patients who arrive before their appointment may still have to wait until their originally scheduled time to be seen.
This means that patients who have an appointment before yours will be seen first, even if they arrive in our lobby/finish their paperwork after you.
We do this to preserve the integrity of every patient's appointment.
In order to maintain patient safety, state and federal laws require us to conduct random drug screenings.
If you are automatically chosen for a drug test, please keep the following in mind:
- If a drug test is requested during your visit, leaving the patient or waiting areas will result in an automatic failure.
- If you need water, please feel free to ask any member of our staff.
- For self-pay patients, the cost of a screening is $100.
